I here declare that the particulers are correct and that I also solemly declare that I shall abide by the rules and regulation of the school as amended time to time.

• Please read the last page of this Application Form and Prospectus carefully.

• All entries should br neatly written / typed in English only.

• One application form is application to only one student.

• The following should be submitted along with this form.

• Copy of birth certificate.

• The original transfer certificate.

• Health certificate by a Doctor highlighting the general health of the student (Play Group)

• Two copies of recent passport size photographs of the student.
